When I attack a pirate fleet and destroy it, it's still there. It sometimes takes multiple tries to destroy the same pirate fleet. Why is this?

Sometimes there are several pirate vessels stacked in the same hex. And they don't combine to become a fleet. Because they're pirates after all. There could be a lot of them depending on game setting. In the upper right corner, see if there is a little red icon. That denotes multiple unstacked ships
Upper right corner of what to look for the red icon? I'm familiar with the red spaceship icon that appears directly above stacked ships. This doesn't seem to be the case here.

I believe the file path to be C:\Users\username\Documents\My Games\GalCiv4
From there you should be able to find the saves folder with the related file
