As more spawned from her, this process, continued by her, brought here to an end, and all of her compatriots.

Maybe Azrael got jealous of the other personalities and killed them? After all, Eddy says Azrael made the other personalities go away. It seems like it would be in character with how he's acted. After all, he claims he's an all-powerful angel, so it fits that he'd deem himself worthy to be Eddy's only creation.
